Suspected of Kikuchi-Fujimoto Disease Presenting with Aseptic Meningitis

Abstract
Kikuchi-Fujimoto disease (KFD), Kikuchi disease or histiocytic necrotizing lymphadenitis affects predominantly young women and have a benign and self-limited course. The involvement of the central nervous system is very rare. But in our case, a 37-year-old man presented with a severe headache. He have a mild fever (37.3¡É). He had the swellings of cervical lymph node developed 1 month ago. The neurologic examinations were unremarkable including signs of meningeal irritation. After admission, the MRI of the brain showed a remarkable diffuse leptomeningeal enhancement. Cerebrospinal fluid analysis revealed an opening pressure 39 cmH2O, 64 white blood cells/mm3, glucose 61 mg/dL and protein 75.6 mg/dL. But, the patient refused a neck biopsy and could not be performed. The patient was diagnosed as having aseptic meningitis and was treated with nonsteroidal anti-inflammatory drugs and corticosteroid. Based on clinical features, we report a patient with suspected of KFD associated by aseptic meningitis. It is an important that recognition of aseptic meningitis with suspected of KFD patients in clinical practice with severe headache, mild fever and elevated erythrocyte sedimentation rate (ESR) even without meningeal irritation signs, suggesting that a lack of specific signs associated with aseptic meningitis.
